SIS Requires Additional Information

Due to a number of additional fields required by the Student Information System when defining a course and scheduling a class, new temporary supplemental forms have been developed. The links on the left will allow you to download the two supplemental forms needed to complete your course package. These are to be edited in MS Word on your local computer, and then attached to the course package in ECA just as you would attach the syllabus or other supporting documents.
